When the flaps were selected down during the approach to Halifax, they traveled to the 30 degree position, stopped and a flap caution message appeared. Caught fire after a third aborted takeoff from John Wayne Airport. Takeoff was twice aborted due to an engine warning light. All three attempts were made within the space of 20 minutes. 2 crew + 6 passengers were uninjured Headed Denver Colorado. Not unexpectedly, the brakes caught fire and the aircraft burnt after left main gear tire overheated and blew on the 3rd takeoff attempt, severing a hydraulic line.

Douglas C54/DC4 had departed from Yellowknife, and was about 15 miles NW when the number 2 engine (Pratt & Whitney-USA R2000-7M2) began to run rough and backfire. After securing the engine, the crew requested a return to Yellowknife and landed without incident. Nil Maintenance found that the number 3 cylinder had cracked around the spark plug hole; replaced the cylinder assembly and released the aircraft back to service.

Dassault Falcon 10 of NovaJet After takeoff from Montreal en route to Winnipeg gear would not retract; aircraft proceeded to Ottawa to burn fuel down to landing weight. The nose steering cable was improperly reconnected by the FBO in Montreal. Nil Line service agent attempted to reconnect the cable but did not lock the cannon plug in place by moving it over center, and as such it was not evident to the crew. As a long term corrective action plan, company has lock wired this cannon plug so it can not be tampered with.

(Jakarta CGK to Malang) 5 passengers injured/97 onboard The 737 was on a VOR approach to runway 35 when the pilots reported an unlocked nose gear. The next day Mandala grounded all 737-200 operations due to “safety concerns.”

767-300ER of SkyMark A food trolley, weighing in at 44 kilograms, caused a shin fracture on one 44-year-old male passenger, while bruising another male passenger’s left shoulder when it crashed into the passengers’ seats.  1 serious injury + 1 minor injury Regulations say trollies must be securely fastened or stowed for landing, but the cabin crew aboard were quoted saying they "did not remember" whether or not proper precautions for securing the trolley had been followed or not.

Spokesman Tommy Vietor said that the mistake on the flight from Chicago made Obama about an hour late for his rally at a community college. On take-off the RH engine separated from the wing after its thrust reverser buckets (T.R.’s) deployed. Aircraft continued to climb out and re-landed at Cape Town without further incident. Six aircraft were diverted to George due to debris on the runway. NTSB report CHI89MA046: Piedmont Airlines N242US, a B737-200, 20Jan89, right engine separated from aircraft after T/O at Chicago. The engine is attached to the pylon by two cone bolts in vibration damper mounts forward and one cone bolt aft. The aft mount also contains a second safety backup bolt. Just above the T.R. is a decal with a green line and an arrow pointing at it, which shows if the rear mount has come loose. If the rear mount is defective (and the engine is only held by the backup bolt), the arrow will not align with the green line (some say the single aft cone bolt has a cable as a back-up, not another bolt).

Aircraft had just landed. On the ramp, ground personnel observed the presence of fire in wheels number 3 and 7. Fire was extinguished using a portable extinguisher. Nil Montréal/Tureau (CYUL) and Frankfurt (EDDF) in Germany. Fire would have been caused by the presence of excessive grease combined with a high temperature of the brakes. No damage was observed.

Antonov AN-12 of Juba Air Cargo The aircraft was destroyed when it crashed on landing outside Khartoum (civil airport) and caught fire. The An-12 collided with army tanks where two soldiers suffered fatal injuries. One other soldier suffered severe injuries. A further 9 people were killed when a fire engine from town rushed to the scene. In their rush to get to the airport, they pushed a bus off the road 2 dead/1 injured Earlier the pilots reported a technical problem (bird strike-kite hawk) soon after take off and turned around for an emergency landing. Carrying 11 tons of cargo. One engine failed shortly after takeoff, due to bird strike - according to the president of Juba Air Cargo. Four crew members, three Russians and one Sudanese, escaped. Overloading is suspected to have played a part.

Flight IB6463 suffered multiple tire bursts on landing at Quito (UIO). The aircraft overran the runway and came to rest tilting to the left with nr.1 and 2 engines touching the ground. It rolled over soft grass before coming to a halt on down-sloping terrain with its left main gear collapsed.
